RANKL (Receptor Activator of Nuclear factor Kappa-B Ligand) is a signaling protein that governs the formation, activity, and survival of osteoclasts, the specialized cells responsible for breaking down bone tissue. Bone is continuously remodeled throughout life through a balance between osteoclasts, which resorb old bone, and osteoblasts, which build new bone. RANKL is produced mainly by osteoblasts and bone marrow stromal cells and acts on a receptor called RANK found on osteoclast precursors, driving their maturation into active bone-resorbing cells. When RANKL signaling is excessive or unopposed, bone breakdown outpaces bone formation, leading to loss of bone density and structural integrity. This imbalance underlies conditions such as osteoporosis, bone loss associated with certain cancers that spread to bone, and skeletal complications arising from hormonal changes or prolonged immobility. Because RANKL sits at a central, rate-limiting point in osteoclast biology, blocking its interaction with RANK offers a way to reduce bone resorption broadly, regardless of the underlying trigger. This makes RANKL an important pharmacological target across multiple diseases characterized by pathological bone loss, and antibody-based therapies have been developed to neutralize it directly before it can engage its receptor.
