Oral PI3K-delta inhibitor

PI3K-delta (phosphoinositide 3-kinase delta, or p110δ) is one of several catalytic subunits of class I PI3-kinase, an enzyme family that converts a lipid messenger, PIP2, into PIP3 at the cell membrane. This lipid signal recruits and activates AKT and downstream effectors that govern cell survival, proliferation, and migration. Unlike the alpha and beta isoforms of PI3K, which are expressed broadly across tissues and support basic metabolic signaling (including insulin action), the delta isoform is enriched in leukocytes, where it operates downstream of the B-cell receptor, T-cell receptor, and various cytokine receptors. This restricted expression makes PI3K-delta an attractive target for diseases driven by dysregulated lymphocyte signaling, including B-cell malignancies, where excessive PI3K-delta activity sustains malignant cell survival, and autoimmune or inflammatory conditions, where it shapes the balance between effector and regulatory T-cell populations. Small-molecule inhibitors that occupy the enzyme's ATP-binding site block this signaling with relative selectivity for immune cells, sparing much of the metabolic signaling handled by other PI3K isoforms. The mechanism is used both to suppress malignant or pathogenic lymphocyte proliferation and, at lower exposures, to recalibrate immune regulatory cells rather than broadly eliminate them.
