Rose Bengal disodium is a photosensitizing xanthene dye formulated into a topical hydrogel that is applied directly onto skin lesions. Photosensitizers are compounds that, upon absorbing light of a defined wavelength, move into an excited electronic state and transfer that energy to molecular oxygen, generating reactive oxygen species such as singlet oxygen. This chemistry underlies photodynamic therapy, an approach in which a photosensitizer is concentrated in a target tissue and then activated by illumination to produce localized oxidative stress while sparing tissue that is not illuminated. Formulating the dye as a hydrogel keeps it confined to the treated area, and controlling the light source restricts activation to the illuminated field, so the resulting reactive oxygen burst occurs only where both compound and light coincide. Depending on dose and depth of activation, the oxidative effect can either destroy diseased cells outright, as historically explored in oncology, or trigger sublethal signaling that modulates local immune and inflammatory activity, a rationale being explored in inflammatory skin diseases such as psoriasis and atopic dermatitis. The general appeal of this mechanism is that it pairs a chemically inert precursor with an externally controlled trigger, limiting systemic exposure and permitting repeated, spatially precise dosing.
