Phosphate binders are agents taken with meals that limit absorption of dietary phosphate from the gastrointestinal tract. In chronic kidney disease (CKD), the kidneys progressively lose the capacity to excrete phosphate, leading to hyperphosphatemia (abnormally high blood phosphate). This drives a cascade known as CKD–mineral bone disorder: elevated phosphate stimulates parathyroid hormone release, promotes calcification of blood vessels and soft tissue, and disrupts bone remodeling through its effects on calcium and vitamin D balance. Because dietary phosphate must first cross the intestinal wall before it can raise blood levels, compounds that bind phosphate ions within the gut and pass through unabsorbed can lower serum phosphate without needing to enter the bloodstream themselves. This category includes non-absorbed polymer resins and metal-based complexes, such as iron- or calcium-containing compounds, that trap phosphate through ion exchange or surface binding. The specific chemistry chosen matters: some binders deliver additional cations like calcium or iron that carry their own effects, while purely organic resins avoid adding any systemic ion load. Phosphate binders serve as a supportive therapy across the spectrum of CKD, including in patients receiving dialysis, where phosphate tends to accumulate because of both dietary intake and diminished renal clearance.
