Peritoneal ultrafiltration is the process by which excess fluid is drawn out of the bloodstream and tissues during peritoneal dialysis, a treatment used when the kidneys can no longer adequately remove water and waste. The peritoneum, a thin membrane lining the abdominal cavity, acts as a natural filter. A dialysis solution instilled into the abdominal cavity contains an osmotic agent—a dissolved substance that creates a concentration gradient—which draws water across the peritoneal membrane from the blood into the dialysis fluid. This fluid, along with the water it has accumulated, is then drained and discarded. The biological rationale is to substitute for lost kidney function in patients with fluid overload from kidney failure, allowing fluid balance to be maintained without relying on blood filtration through an external machine. Different osmotic agents achieve this gradient differently: some rely on small sugar molecules that generate a strong but short-lived gradient, while others use larger, slowly absorbed polymers that sustain fluid removal over longer dwell times. The choice of agent affects how much fluid is removed, how long the effect lasts, and how the solution interacts with the body's metabolism and the peritoneal membrane itself over repeated use.
