PD-1 (Programmed Cell Death Protein 1) is an inhibitory receptor expressed on activated T cells that normally functions as a brake on immune responses, limiting how vigorously T cells proliferate and produce inflammatory signals once an antigen has been recognized. This receptor is best known from oncology, where blocking it releases T cells to attack tumors. The opposite strategy—agonizing, or deliberately strengthening, PD-1 signaling—aims to reinforce that natural brake rather than release it. In autoimmune and inflammatory diseases, T cells that mistakenly target the body's own tissues remain active in part because inhibitory checkpoints like PD-1 fail to sufficiently restrain them. A PD-1 agonist is designed to re-engage this inhibitory pathway, calming autoreactive T cells and reducing the tissue damage they cause, potentially across a range of autoimmune conditions where broad or targeted immune dampening is desirable. Because PD-1 is enriched on T cells that are currently engaged in an active response, agonizing it may preferentially affect cells contributing to ongoing disease rather than the entire immune repertoire, offering a more selective alternative to broadly immunosuppressive therapies. This mechanism represents an emerging approach distinct from conventional immunosuppressants, built on decades of understanding of how checkpoint biology governs the balance between immune defense and self-tolerance.
