Eicosapentaenoic acid (EPA) is a long-chain omega-3 polyunsaturated fatty acid obtained from marine sources or synthesized pharmaceutically as a purified ethyl ester. When incorporated into cell membranes and circulating lipoproteins, it influences several interconnected lipid and inflammatory pathways rather than acting through a single receptor. Its principal pharmacological rationale is to lower elevated triglycerides, a lipid fraction associated with cardiovascular risk, by reducing hepatic production of triglyceride-rich very-low-density lipoprotein (VLDL) particles and by enhancing the enzymatic clearance of triglycerides from the bloodstream. EPA also competes with arachidonic acid, another fatty acid that serves as the precursor for many pro-inflammatory signaling molecules, thereby shifting the balance of locally produced lipid mediators toward less inflammatory and pro-resolving forms. This combination of triglyceride-lowering and anti-inflammatory activity has made omega-3 fatty acids of interest in cardiovascular disease, particularly in patients with persistently high triglycerides despite standard lipid-lowering therapy. Because the mechanism involves multiple, modest, and partially overlapping pathways rather than a single potent target, purified EPA formulations are generally used as adjuncts to, rather than replacements for, other lipid-modifying therapies.
