Myostatin, also called GDF8 (growth differentiation factor 8), is a signaling protein secreted primarily by skeletal muscle that acts as a natural brake on muscle growth. It belongs to the transforming growth factor-beta (TGF-beta) superfamily of growth factors. By binding to activin type II receptors on muscle cells and triggering an intracellular signaling cascade, myostatin limits the proliferation of muscle precursor cells and restrains protein synthesis, keeping muscle mass within a physiological range. Blocking myostatin, whether with a neutralizing antibody, a ligand trap, or another biologic, removes this restraint and allows muscle tissue to grow beyond its normal set point. This rationale has been explored across conditions marked by muscle loss, including muscular dystrophies, age-related sarcopenia, and cancer-associated wasting (cachexia). A newer application pairs myostatin inhibition with incretin-based weight-loss therapies, such as GLP-1/GIP receptor agonists, because these agents reduce not only fat but also lean muscle mass during weight loss. Adding a myostatin-blocking agent is intended to preserve or build muscle while the metabolic drug drives fat loss, aiming for a healthier overall change in body composition rather than a strategy focused only on total weight reduction.
