Melanocortin-4 receptor agonist

Melanocortin-4 receptor (MC4R) is a G-protein-coupled receptor concentrated in the hypothalamus, the brain region that governs appetite and energy expenditure. It sits downstream of leptin signaling: leptin, released from fat tissue, activates neurons that produce pro-opiomelanocortin (POMC), which is processed into melanocortin peptides that bind and activate MC4R. This activation suppresses hunger and increases energy use. When any step in this leptin-melanocortin pathway is disrupted—through genetic mutations affecting the leptin receptor, POMC processing, or MC4R itself—the result is a syndrome of severe, early-onset obesity driven by persistent hunger rather than by lifestyle factors. Synthetic MC4R agonists are designed to bypass upstream defects and directly activate the receptor, restoring the satiety signal. This approach is most rationally applied in genetically defined obesity syndromes where the pathway is known to be broken, though the same biology has broader relevance to appetite regulation in obesity generally. Because MC4R also has roles outside appetite control, agonism can produce effects unrelated to weight, which is a central consideration in how these agents are developed and dosed.
