Leukocyte immunoglobulin-like receptor B (LILRB) family members are inhibitory receptors expressed mainly on myeloid cells—monocytes, macrophages, and dendritic cells—as well as some lymphocytes. Structurally related to natural killer cell inhibitory receptors, they carry intracellular motifs that dampen activating signals when engaged by their ligands, which include certain major histocompatibility complex class I molecules and other proteins found on tumor cells or in inflamed tissue. This inhibitory signaling helps maintain immune tolerance under normal conditions, but tumors can exploit it to blunt the activity of myeloid cells that would otherwise present antigens, secrete inflammatory mediators, or support cytotoxic immune responses. Blocking LILRB signaling is therefore being explored as a way to relieve this brake on myeloid and broader immune function, shifting the tumor microenvironment toward a more activated, pro-inflammatory state. This mechanism sits within the broader category of myeloid-targeted immuno-oncology strategies, which aim to complement T-cell-focused checkpoint approaches by re-engaging innate immune effector cells.
