The leptin receptor is a cell-surface protein expressed predominantly in the hypothalamus, the brain region that regulates energy balance. Its natural ligand, leptin, is a hormone secreted by fat tissue in proportion to the body's energy stores. When leptin binds its receptor, it signals to the brain that energy reserves are adequate, suppressing appetite and permitting normal metabolic and reproductive function. In conditions where the body produces little or no leptin, or where fat tissue itself is deficient (as in certain lipodystrophies), this satiety signal is absent, leading to persistent hunger and metabolic disturbances such as severe insulin resistance and abnormal lipid handling. An agonist antibody directed at the leptin receptor is designed to substitute for the missing hormonal signal, activating the receptor directly to restore downstream appetite-suppressing and metabolic signaling. This approach is relevant primarily in rare disorders of leptin deficiency or absence, where replacing the missing signal can meaningfully correct the underlying physiology, rather than in common obesity, where leptin levels are typically already elevated and the brain has become resistant to the hormone's effects.
