This mechanism targets three signaling proteins central to type 2 immune inflammation: interleukin-33 (IL-33), an "alarmin" released by damaged or stressed tissue lining (epithelium), and interleukin-4 (IL-4) and interleukin-13 (IL-13), the effector cytokines that IL-33 helps trigger. When epithelial cells in the airway, skin, or gut are irritated by allergens, pathogens, or physical injury, they release IL-33, which activates innate and adaptive immune cells to produce IL-4 and IL-13. These two cytokines then drive the hallmark features of type 2 inflammation: mucus overproduction, airway or skin barrier dysfunction, eosinophil recruitment, and immunoglobulin E (IgE) production. Diseases in this family include asthma, atopic dermatitis, chronic rhinosinusitis, and related allergic or fibrotic conditions. A trispecific antibody that neutralizes IL-33, IL-4, and IL-13 simultaneously is designed to interrupt the pathway at two levels at once: the upstream alarmin signal that initiates the response, and the downstream effector cytokines that execute it. This dual-level blockade aims to achieve broader suppression of the inflammatory cascade than an agent targeting only one node, particularly in patients whose disease is driven by multiple, overlapping arms of this signaling network.
