Interleukin-1 alpha (IL-1α) is a cytokine belonging to the interleukin-1 family, a group of signaling proteins that coordinate the body's innate inflammatory response. Unlike many cytokines that are synthesized only after a triggering signal, IL-1α is constitutively present in many cell types, stored in an active form that can be released rapidly when cells are damaged or die, making it an "alarmin" that alerts the immune system to tissue injury. Once released, it binds the same receptor complex used by its relative IL-1 beta, triggering downstream signaling that amplifies production of other inflammatory mediators. Blocking IL-1α with a monoclonal antibody is intended to dampen this early, damage-triggered wave of inflammation without necessarily affecting the inflammasome-dependent pathway that generates IL-1 beta. This distinction matters in conditions where tissue injury, rather than a classic infection-driven inflammasome trigger, is the dominant driver of inflammation, such as certain autoinflammatory and inflammatory skin conditions, cardiovascular disease linked to vascular injury, and tumor microenvironments where chronic sterile inflammation supports disease progression. Targeting IL-1α offers a way to interrupt this alarm signal at its source, upstream of the broader inflammatory cascade it helps set in motion.
