The histamine H1 receptor is a G protein-coupled receptor found on smooth muscle, vascular endothelium, and neurons, including cells in the central nervous system. When histamine, a signaling molecule released mainly by mast cells and basophils, binds this receptor, it triggers downstream signaling that increases vascular permeability, contracts smooth muscle in the airways and gut, and promotes wakefulness and sensory signaling such as itch. This activity underlies many features of allergic reactions, including nasal congestion, hives, and bronchoconstriction, as well as normal regulation of sleep-wake cycles. Small molecules that block the H1 receptor, known as H1-antihistamines, prevent histamine from activating it, dampening these downstream effects. This class of drugs has broad relevance across allergic and inflammatory conditions, including seasonal allergies, urticaria (hives), and some itch-related skin conditions. Beyond classic allergy applications, H1 receptor blockade is also being explored for its effects on neuronal excitability, since the receptor is expressed in brain regions involved in seizure threshold and arousal, giving this mechanism relevance to neurological research contexts as well. Because H1 receptors exist both peripherally and centrally, the therapeutic profile of a given antihistamine depends heavily on how much it penetrates the brain.
