The growth hormone-releasing hormone (GHRH) receptor is a cell-surface receptor found on somatotroph cells in the anterior pituitary gland. When its natural hormone, GHRH, binds this receptor, it triggers the synthesis and pulsatile release of growth hormone (GH) into the bloodstream. GH in turn prompts the liver to produce insulin-like growth factor 1 (IGF-1), a hormone central to tissue growth, protein synthesis, and metabolic regulation, including fat and glucose handling. Modulating this receptor with a synthetic GHRH analog offers a way to raise GH and IGF-1 levels by working through the body's own regulatory machinery, rather than by administering GH directly. Because pulsatile, receptor-driven release preserves the natural feedback loops that govern hormone levels, this approach can restore more physiological patterns of GH secretion. This mechanism is broadly relevant in conditions marked by GH deficiency or dysregulated GH-axis signaling, and in metabolic disturbances involving abnormal fat distribution, where increasing endogenous GH pulsatility can favorably shift fat metabolism. The rationale for using a receptor agonist rather than replacing GH itself rests on maintaining the body's intrinsic checks on hormone output, though downstream effects on IGF-1 and metabolism still require careful clinical attention.
