Growth hormone receptor (GHR) is a cell-surface receptor found mainly in the liver, cartilage, muscle, and fat tissue. Its natural partner, growth hormone (GH), is released from the pituitary gland in pulses and coordinates growth during childhood as well as ongoing metabolic balance in adults. Recombinant human growth hormone, or somatropin, is a biologic manufactured to have the same amino acid sequence as natural GH. It acts as a direct agonist at GHR, meaning it binds and activates the receptor the way the body's own hormone would, which is useful when a person's natural GH production or signaling is insufficient. Activation of GHR triggers an intracellular signaling cascade that, notably, causes the liver to produce insulin-like growth factor 1 (IGF-1), the molecule responsible for much of GH's effect on bone lengthening and tissue growth. This mechanism is central to treating growth hormone deficiency in children and adults, certain genetic conditions causing short stature, and states of tissue wasting where preserving muscle and metabolic function matters. Because GHR signaling simultaneously influences growth, glucose regulation, and fat metabolism, therapeutic use aims to restore physiological hormone levels rather than to saturate the receptor beyond what the body would normally experience.
