GPCR19, also known as GPBAR1 or TGR5, is a G protein-coupled receptor activated naturally by bile acids, molecules produced from cholesterol that help digest fats but also act as signaling molecules throughout the body. TGR5 is expressed not only in the liver and gut but also in immune cells and skin cells, where its activation dampens inflammatory signaling pathways, including NF-κB (a master switch for inflammatory gene expression) and the inflammasome (a protein complex that triggers release of inflammatory cytokines). Because chronic activation of these pathways drives many inflammatory skin conditions, a compound that turns on TGR5 locally in skin tissue has the potential to reduce redness, itching, and tissue damage associated with such conditions. Formulating a TGR5 agonist as a topical gel, rather than an oral or injectable therapy, is a strategy to concentrate its anti-inflammatory effect at the skin surface while limiting exposure elsewhere in the body, since TGR5 also participates in bile acid and glucose regulation systemically. This approach reflects a broader principle in inflammatory disease treatment: engaging a biologically validated anti-inflammatory pathway while confining drug action to the affected tissue to minimize effects on unrelated organ systems.
