Glucokinase is an enzyme found mainly in pancreatic beta cells and liver cells, where it catalyzes the first committed step of glucose metabolism: phosphorylation of glucose to glucose-6-phosphate. Because the rate of this reaction depends on how much glucose is present, glucokinase functions as a cellular glucose sensor, setting the threshold at which insulin is released from the pancreas and at which the liver switches between storing and releasing glucose. In type 2 diabetes, this sensing is blunted, contributing to poor blood sugar control. Glucokinase activators are small molecules that bind the enzyme and increase its sensitivity to glucose, effectively lowering the glucose concentration needed to trigger insulin secretion and hepatic glucose uptake. This dual site of action—pancreas and liver—is central to the rationale for the drug class, since correcting insulin secretion alone does not address the liver's contribution to elevated blood sugar. Different molecules in this class vary in how strongly they act on each tissue, which shapes their potential benefits and risks. This mechanism has been explored broadly for type 2 diabetes and, in some cases, for rarer genetic forms of glucose intolerance linked to glucokinase mutations.
