This combination pairs two mechanisms commonly used together in chronic airway disease: an inhaled corticosteroid (ICS) acting on the glucocorticoid receptor, and a long-acting muscarinic antagonist (LAMA) blocking the M3 acetylcholine receptor. The glucocorticoid receptor is a nuclear receptor that, once bound by a corticosteroid, moves into the cell nucleus and alters the transcription of genes involved in inflammation, generally dampening production of inflammatory proteins and immune cell recruitment in the airway lining. The M3 receptor is a G-protein-coupled receptor on airway smooth muscle that, when activated by the neurotransmitter acetylcholine, triggers muscle contraction and narrows the airway. Blocking M3 with an antagonist prevents this contraction, keeping airways more open. Combining the two addresses two distinct contributors to obstructive lung disease at once: ongoing tissue inflammation and reflex-driven muscle tone. This dual approach is relevant to chronic obstructive pulmonary disease (COPD) and, in related combinations, asthma, where both inflammation and bronchoconstriction contribute to breathlessness and airflow limitation. Using an ICS and a LAMA together, rather than either alone, is intended to produce more comprehensive symptom control than targeting only inflammation or only muscle tone, reflecting the multifactorial nature of these diseases.
