The glucocorticoid receptor (GR) is a nuclear hormone receptor found in nearly all cell types, including the keratinocytes and immune cells that reside in skin. When bound by a corticosteroid such as hydrocortisone, GR moves into the nucleus and changes the transcription of genes that govern inflammation, reducing production of cytokines, prostaglandins, and other mediators responsible for redness, swelling, and itch. This anti-inflammatory action underlies the use of topical corticosteroids in inflammatory skin diseases such as eczema, psoriasis, and contact dermatitis. Corticosteroids alone, however, do not correct the impaired skin barrier that often underlies and perpetuates these conditions: loss of lipids in the outermost skin layer increases water loss and allows easier penetration of irritants and allergens, which sustains inflammation. Pairing a corticosteroid with a barrier-repair component, such as a lipid or sulphated polysaccharide preparation that helps restore this protective layer, aims to interrupt the itch-inflammation cycle from two directions at once: suppressing immune signaling while rebuilding the physical barrier. This combined approach is broadly relevant to chronic inflammatory skin conditions in which barrier dysfunction and immune activation reinforce one another.
