The farnesoid X receptor (FXR, gene name NR1H4) is a nuclear receptor, a type of protein that resides mostly inside liver and intestinal cells and, when activated, moves to the nucleus to switch specific genes on or off. Its natural activators are bile acids, molecules made from cholesterol that help digest dietary fat. FXR acts as a sensor: when bile acid levels rise, FXR activation reduces further bile acid production and adjusts how the body handles cholesterol, triglycerides, and glucose. This places FXR at the center of bile acid and lipid metabolism signaling. Because disrupted bile acid handling and lipid overload contribute to chronic liver diseases such as fatty liver disease with inflammation and scarring, and to cholestatic conditions where bile flow is impaired, synthetic small-molecule agonists that mimic and amplify bile acid signaling at FXR have been developed. The rationale is to reduce bile acid accumulation, dampen liver inflammation and fibrosis, and improve metabolic parameters, offering a mechanistic approach across a range of chronic liver conditions where the underlying biology involves bile acid or lipid dysregulation.
