EP2 and EP4 are two related receptors for prostaglandin E2 (PGE2), a lipid signaling molecule produced at sites of tissue damage, inflammation, and within tumors. Both receptors are G-protein-coupled receptors that, when activated by PGE2, raise levels of the intracellular messenger cyclic AMP, which in turn dampens the activity of immune cells such as T cells, natural killer cells, and macrophages. Tumors frequently produce large amounts of PGE2 to exploit this pathway, creating a local environment that suppresses anti-tumor immune responses. An EP2/EP4 antagonist blocks PGE2 from engaging either receptor, aiming to remove this brake on immune cell function and restore the capacity of the immune system to recognize and attack malignant cells. Targeting both receptors simultaneously, rather than just one, addresses the fact that PGE2 signals through overlapping and partially redundant pathways via EP2 and EP4, so blocking a single receptor may leave sufficient residual signaling to sustain immune suppression. This mechanism is of broad interest in oncology, particularly for tumors that rely heavily on prostaglandin-driven immune evasion, and is generally explored alongside other immune-modulating approaches rather than as a standalone strategy.
