Type 1 regulatory T (Tr1) cells are a specialized subset of CD4+ T lymphocytes that dampen immune responses mainly by secreting the anti-inflammatory signaling protein interleukin-10 (IL-10), distinguishing them from conventional regulatory T cells (Tregs) that depend on the transcription factor FoxP3. Engineered Tr1 cell therapies aim to harness this suppressive function to restore immune balance in conditions driven by excessive or misdirected immune activity, including autoimmune disease, graft-versus-host disease after stem cell transplantation, and rejection of transplanted organs. Because Tr1 cells can be directed to recognize particular antigens, they offer the possibility of antigen-focused suppression that dampens a specific harmful immune response rather than immunity broadly. Engineering these cells allows large-scale expansion outside the body, introduction or selection of a defined antigen specificity, and use of donor-derived (allogeneic) starting material to produce a standardized, off-the-shelf cell product rather than one manufactured individually for each patient. This mechanism belongs to a broader class of cell-based immunotherapies that seek to reprogram immune regulation rather than simply block it, with the aim of achieving durable tolerance that could lessen dependence on ongoing immunosuppressive drug treatment.
