This mechanism describes a multimodal receptor profile designed to modulate both dopaminergic and serotonergic signaling, two neurotransmitter systems implicated in psychotic and mood disorders. The molecule acts as a partial agonist at dopamine D2, D3, and D4 receptors, meaning it can dampen dopamine signaling where it is excessive while providing a degree of baseline stimulation where dopamine tone is low. It shows the same partial-agonist behavior at serotonin 5-HT1A and 5-HT2A receptors, subtypes long linked to mood regulation, anxiety, and the motor and cognitive side effects seen with older antipsychotics. In addition, it blocks (antagonizes) serotonin 5-HT2B, 5-HT6, and 5-HT7 receptors, subtypes thought to contribute to cognitive function, mood, and sleep-wake regulation. The rationale for combining these activities in one molecule, rather than targeting a single receptor, is to address the broad symptom domains of schizophrenia and related disorders, including hallucinations and delusions, blunted affect, and cognitive impairment, while attempting to minimize the movement disorders and hormonal disturbances associated with drugs that fully block dopamine receptors. This general strategy underlies most modern atypical antipsychotics and is also explored in mood and cognitive disorders where dopamine-serotonin balance is disrupted.
