The D2 and D3 dopamine receptors belong to the D2-like family of G-protein coupled receptors, which are activated by the neurotransmitter dopamine and are concentrated in brain regions that govern movement, mood, and reward, as well as in the pituitary gland. Dopamine signaling through these receptors is central to Parkinson's disease, in which the neurons that supply dopamine to motor circuits progressively degenerate, producing tremor, rigidity, and slowed movement. Drugs that engage D2/D3 receptors can substitute for the lost dopamine tone in these circuits. A partial agonist occupies the receptor and produces a submaximal response, meaning it provides a floor of stimulation when natural dopamine is scarce but tempers signaling when dopamine is abundant. This behavior is intended to smooth out the fluctuating receptor activity that contributes to motor complications seen with some dopaminergic therapies. Combined D2/D3 activity is chosen because these two subtypes work together in the striatal and limbic circuits responsible for motor control and reward processing, so engaging both can address movement symptoms while reflecting the receptor distribution naturally used by dopamine itself. This mechanism broadly matters wherever restoring or modulating dopaminergic tone is therapeutically relevant, most notably in movement disorders.
