The mechanism targets both D1 and D2 dopamine receptors, the two principal receptor subtypes through which dopamine acts in the brain, particularly within circuits of the basal ganglia that govern movement, motivation, and reward. In conditions such as Parkinson's disease, loss of dopamine-producing neurons reduces stimulation of both receptor types, disrupting the balance between the brain's 'direct' and 'indirect' motor pathways, which are preferentially linked to D1 and D2 receptors respectively. Most dopamine-replacement therapies to date have relied on receptors sensitive mainly to one subtype, D2-like receptors, leaving the D1-dependent pathway comparatively understimulated. A dual D1/D2 agonist is designed to engage both pathways simultaneously, more closely approximating the pattern of receptor activation produced by dopamine itself. The rationale is that combined stimulation may produce more balanced, physiological motor circuit activity than single-pathway stimulation, potentially improving control of movement symptoms. This dual-target approach is of broad interest in movement disorders and is also studied in contexts where dopaminergic signaling influences mood, cognition, and motivation. The overall aim is to restore a more complete dopaminergic tone rather than partially compensating for its loss.
