CIDEB (Cell Death-Inducing DFFA-like Effector B) is a hepatocyte protein belonging to the CIDE family, which also includes CIDEA and CIDEC. These proteins govern the fusion and enlargement of lipid droplets, the intracellular organelles that store triglycerides. CIDEB additionally participates in packaging triglycerides into very-low-density lipoprotein (VLDL) particles for export from the liver into the bloodstream. Human genetic studies have identified naturally occurring loss-of-function variants in CIDEB that are associated with lower liver fat content and a reduced risk of progressive liver disease, including steatohepatitis and fibrosis, without evident harm. This human validation provides the biological rationale for developing agents that reduce CIDEB activity or expression: doing so is intended to mimic the protective genetic state pharmacologically. The broad relevance of this mechanism lies in chronic metabolic liver disease, where excess accumulation of fat within hepatocytes can progress through inflammation and scarring toward cirrhosis. By limiting the growth of lipid droplets and associated lipotoxic stress, modulation of CIDEB is being explored as a way to intervene upstream of this progression, targeting a liver-restricted pathway of lipid handling rather than systemic lipid metabolism broadly.
