CDK8 and CDK19 are closely related enzymes that associate with cyclin C to form the kinase module of the Mediator complex, a large protein assembly that helps regulate how genes are turned into RNA transcripts. Unlike the better-known cyclin-dependent kinases that drive cell division, CDK8 and CDK19 act mainly as transcriptional regulators, fine-tuning the activity of transcription factors involved in growth signaling, immune signaling, and developmental pathways. Because the two kinases are functionally redundant, inhibitors are typically designed to block both simultaneously; targeting only one leaves the other able to compensate. In certain cancers, particularly acute myeloid leukemia and related blood cancers, this Mediator kinase module supports transcriptional programs that maintain leukemic stem cells in an undifferentiated, self-renewing state. Blocking CDK8/CDK19 activity can disrupt these programs, encouraging malignant cells to differentiate into more mature, less proliferative forms or become more susceptible to other treatments. This mechanism is of broad interest across hematologic malignancies and is also being explored in solid tumors where similar transcriptional dependencies exist. Because Mediator kinase activity intersects with normal developmental and immune signaling pathways, understanding its selective role in disease versus healthy tissue is central to evaluating this drug class.
