CD138, also known as Syndecan-1, is a cell-surface heparan sulfate proteoglycan expressed at high density on plasma cells, including the malignant plasma cells of multiple myeloma, making it a well-established marker and targeting handle for this disease. The mechanism described here uses an antibody directed against CD138 fused to a modified, or 'masked,' form of interferon-alpha, a cytokine that activates innate and adaptive immune responses and can directly slow proliferation of susceptible cells. Interferon-alpha is a potent but systemically toxic signaling molecule when administered on its own, since it engages its receptor throughout the body and produces flu-like symptoms and blood cell suppression. By fusing it to an antibody and physically masking its active site until the construct reaches its target, the goal is to concentrate interferon activity at sites of CD138-expressing tumor cells while limiting exposure elsewhere. This class of molecule, called a masked immunocytokine, aims to combine the tumor-homing precision of antibody therapy with the immune-activating potency of cytokine therapy, addressing diseases such as multiple myeloma where broad cytokine administration has historically been limited by toxicity rather than lack of efficacy.
