Cardiac troponin C is a calcium-binding regulatory protein within the troponin complex that sits on cardiac muscle thin filaments. When calcium binds troponin C during each heartbeat, it triggers a conformational change that permits actin and myosin to interact, producing contraction. A calcium sensitizer increases the affinity of troponin C for calcium, so that a given amount of calcium produces a stronger contraction than it otherwise would. This is pharmacologically distinct from traditional inotropes (such as catecholamines or phosphodiesterase inhibitors), which increase contractility mainly by raising intracellular calcium concentration itself. Because sensitizers work downstream of calcium release, they can improve pump function without proportionally raising the cell's calcium load, which is relevant because excess intracellular calcium is linked to arrhythmia risk and increased oxygen consumption. Some agents in this class also open ATP-sensitive potassium (KATP) channels in vascular smooth muscle, producing vasodilation that reduces the resistance the heart pumps against. This combination is of particular interest in acute decompensated heart failure and cardiogenic shock, where clinicians want to support cardiac output without further stressing an already oxygen-starved heart muscle.
