Cannabidiol (CBD) acts on the endocannabinoid system, a signaling network built around two receptors, CB1 and CB2, that normally respond to naturally occurring lipid messengers called endocannabinoids. CB1 receptors are concentrated on nerve endings throughout the brain and spinal cord, where their activation dampens the release of neurotransmitters; CB2 receptors are found mainly on immune cells and influence inflammatory activity. Unlike tetrahydrocannabinol (THC), CBD binds these receptors only weakly and instead influences the system indirectly, through several other molecular targets that affect neuronal excitability, calcium handling, and inflammatory signaling. This broad, low-potency, multi-target profile is the basis for interest in CBD across conditions where nervous system hyperexcitability or inflammation plays a role, including certain seizure disorders, anxiety states, and chronic pain or inflammatory conditions. Because CBD does not strongly activate CB1, it is not associated with the intoxicating effects typical of THC, which has made it attractive as a cannabinoid-based therapeutic distinct from recreational cannabis constituents. The rationale for developing it as a defined pharmaceutical product, whether as a purified small molecule or a standardized botanical extract, rests on delivering consistent dosing of this multi-target activity rather than relying on variable plant material.
