Autologous dual-target CAR-T cell therapy

This mechanism describes a cell-based therapy for multiple myeloma, a cancer of antibody-producing plasma cells. Autologous CAR-T cell therapy takes a patient's own T cells and re-engineers them, outside the body, to carry a chimeric antigen receptor (CAR) — a synthetic protein that lets the T cell recognize a specific marker on cancer cells and kill them without needing conventional immune recognition machinery. This construct targets two markers found on plasma cells simultaneously: BCMA, a receptor largely restricted to mature plasma cells that supports their survival, and CD38, an enzyme broadly expressed on plasma cells and used by other myeloma therapies. Targeting both markers at once, rather than one alone, addresses a recognized limitation of single-target approaches: tumor cells can reduce or lose expression of one antigen and escape immune attack. A dual-target design increases the chance that a malignant cell expressing either marker is still recognized and eliminated, and may allow killing of cells with lower density of either antigen alone. This class of therapy is relevant wherever plasma cell malignancies recur after initial treatment, particularly when resistance is driven by loss of a single therapeutic target.
