ABCA1 (ATP-binding cassette transporter A1) is a membrane protein that exports cholesterol and phospholipids from the interior of cells to lipid-poor apolipoprotein A-I, the first step in forming high-density lipoprotein (HDL) particles. This process, called cholesterol efflux, is central to reverse cholesterol transport, the pathway by which excess cholesterol in peripheral tissues, including cholesterol-laden macrophages in the artery wall, is returned to the liver for excretion. Because impaired ABCA1 function leads to cholesterol accumulation in macrophages and contributes to atherosclerotic plaque formation, therapeutic strategies have explored ways to enhance ABCA1 activity pharmacologically, aiming to increase efflux capacity and support plaque stabilization or regression. Molecules designed as ABCA1 agonists, including peptide-based agents that mimic or enhance apolipoprotein A-I interactions, are studied for their potential to promote this efflux process. The mechanism is of broad interest in cardiovascular disease, where lipid handling within the vessel wall influences plaque development, and it also has relevance to rare genetic disorders in which ABCA1 dysfunction causes severe HDL deficiency and abnormal lipid deposition in tissues.
