The primary endpoint was the surrogate measure of change in body weight, and although the reported mean reduction favored OPC-61815 numerically, the statistical significance of the between-group comparison is not stated.

This large trial randomly assigned 3,600 people with heart failure to tolvaptan or placebo, with treatment assignment masked. Neither primary outcome favoured tolvaptan: all-cause mortality was 25.9% against 26.3% on placebo (hazard ratio 0.98, 95% CI 0.87 to 1.11, p=0.68), and the combined rate of cardiovascular death or heart-failure hospitalisation was 42.0% against 40.2% (hazard ratio 1.04, 95% CI 0.95 to 1.14, p=0.55). The trial was well built and its answer was that tolvaptan did not change these outcomes.
