Head-to-head evidence
Glycopyrrolate/formoterol fumarate vs Tiotropium bromide
Same mechanism, same race: design-graded trial evidence for both assets, with the grading rationale shown. Grades describe the quality of the evidence, never the merits of either security.

Glycopyrrolate/formoterol fumarate · Bevespi Aerosphere
Marketed by AstraZeneca
NCT03324607 · Phase 2/3 · completed
The study used a single treatment group without randomization, blinding, or a placebo or active comparator, and only 17 participants contributed to the primary imaging analysis. Its primary endpoints were surrogate MRI measures rather than established clinical outcomes, and the findings were mixed across those measures, so they provide limited evidence about Bevespi’s clinical effectiveness.
Tiotropium bromide
Marketed by Boehringer Ingelheim
NCT02172287 · Phase 3 · completed
This is a large, randomized, double-blind trial with both an active comparator and a placebo arm, using trough FEV1 lung function, the regulator-accepted physiological measure for COPD, as one of its primary endpoints. Registry results were not posted at the time of grading; the key result shown was recovered from the cited external source.
