Head-to-head evidence
difamilast vs roflumilast
Same mechanism, same race: design-graded trial evidence for both assets, with the grading rationale shown. Grades describe the quality of the evidence, never the merits of either security.

difamilast · Moizerto / Adquey
Marketed by Aurobindo Pharma and partners
NCT03908970 · Phase 3 · completed
This was a randomized, triple-blind (participant, investigator, and care provider), placebo-controlled Phase 3 trial in 364 adult patients with atopic dermatitis. The primary endpoint was the IGA responder rate at Week 4, which was met: 38.46% (95% CI 31.36–45.95) for 1% OPA-15406 versus 12.64% (95% CI 8.18–18.36) for placebo. The trial was adequately powered and used a validated clinical disease activity measure.

roflumilast · Zoryve
Marketed by Arcutis Biotherapeutics
NCT06631170 · Phase 3 · completed
The trial was randomized, fully blinded, and vehicle-controlled with a validated disease-severity measure as its primary endpoint, and roflumilast cream produced significantly more treatment successes than vehicle (31.9% vs 6.7%, P<0.0001).
