Head-to-head evidence
Blixeprodil vs Zelquistinel
Same mechanism, same race: design-graded trial evidence for both assets, with the grading rationale shown. Grades describe the quality of the evidence, never the merits of either security.

Blixeprodil
Developed by Gilgamesh Pharmaceuticals
NCT06309277 · Phase 2 · completed
The study was randomized, quadruple-blind, placebo-controlled, and included 46 participants, but the primary endpoint was treatment-emergent adverse-event incidence rather than an efficacy endpoint. The registry reports adverse-event counts but no statistical comparison for this primary endpoint, so whether it was met is unclear; the depression-rating result was a statistically significant secondary finding.
Zelquistinel
Developed by Syndeio Biosciences
NCT03726658 · Phase 1/2 · completed
The study randomized 226 participants overall to several zelquistinel dose groups or placebo and masked both participants and investigators, but it stated that no power calculation was performed and each comparison was relatively small. The primary MADRS comparisons were clinical measures but were missed, with all reported p-values above 0.05, so the findings provide limited evidence of benefit.
Next expected readout: September 2027 · NCT06547489 (registry estimate)
