Head-to-head evidence

Baxdrostat vs Vicadrostat

Same mechanism, same race: design-graded trial evidence for both assets, with the grading rationale shown. Grades describe the quality of the evidence, never the merits of either security.

Shared mechanism: Aldosterone synthase (CYP11B2) · full Hypertension pipeline

Reliable signal

NCT06034743 · Phase 3 · completed

The trial randomized 794 participants to baxdrostat or matching placebo and used quadruple masking of participants, care providers, investigators, and outcome assessors. Both prespecified primary blood-pressure endpoints were met at Week 12: the reduction in seated systolic blood pressure was greater with 2 mg and 1 mg baxdrostat than with placebo, with p<0.0001 for each comparison.

Reliable signal

NCT05182840 · Phase 2 · completed

This was a randomized, quadruple-blind, placebo-controlled phase 2 trial in 586 patients with chronic kidney disease, comparing three doses of vicadrostat (3, 10, and 20 mg) against placebo, all on a background of empagliflozin or placebo. The primary endpoint was the change in urine albumin-to-creatinine ratio (UACR), a physiological measure that defines and stages CKD, at 14 weeks; the 10 mg and 20 mg doses achieved significantly larger reductions than placebo (p<0.0001) and the dose-response analysis was significant, so the primary endpoint was met. The 3 mg dose was only borderline (p=0.0499), making the overall result favorable but clearly dose-dependent.
